Position Summary
  • Work as part of the Aged and Disability Services team at GLCH
  • Fixed term - 12 months, part-time

Home Care Packages (HCP) are individually planned and coordinated packages of community aged care services, targeted at frail older people living in the community who require management of services because of their complex care needs.

The Care Manager is responsible for managing a set case load of HCP clients. This involves working closely with each client and their families to ensure they receive an individualised, responsive service under a consumer directed care model.
